What Are Mounjaro and Wegovy Treatments?

Mounjaro and Wegovy are both prescription-only injections that you take once a week. They work similarly, but they are not quite the same.

The MHRA has approved Mounjaro and Wegovy for weight loss in adults, and healthcare professionals can also use tirzepatide and semaglutide to treat type 2 diabetes.

In trials, the weight loss has been striking. People on higher doses of Mounjaro lost about 20% or more of their body weight over about 18 months, while Wegovy produced similar losses at its highest dose.

How Mounjaro and Wegovy Work

These medicines affect your brain and digestive system in ways that reduce your appetite. They cause you to feel full faster, slow your digestion, and regulate your blood sugar. This leads you to eat less food and become thinner.

That weight loss matters for the liver. When the body sheds fat, it usually loses fat from the liver too, and a lighter liver tends to work better.

Does Mounjaro Affect Your Liver?

For most people, yes, and mostly in a helpful way. As you lose weight, the fat inside your liver falls, inflammation settles, and your liver blood tests often improve.

There is a flip side worth knowing about. In the early weeks, some people see a mild, temporary rise in their liver enzymes. Researchers think this can happen as fat moves out of the liver quite quickly. In most cases, these readings settle on their own. Mounjaro and Fatty Liver Disease

The link between Mounjaro and fatty liver disease is one of the most exciting areas in liver medicine right now.

Fatty liver disease, now called MASLD (metabolic dysfunction-associated steatotic liver disease), happens when too much fat builds up in the liver. In some people, it turns into MASH, the more serious, inflamed form that can scar the liver over time.

In a 2024 study known as SYNERGY-NASH, one of these treatments proved superior to placebo. Doctors usually confirm fatty liver and scarring with simple tests. A fibrosis scan and an ultrasound can measure fat and scarring without needles, and clinicians can also include a blood panel.

Semaglutide and Fatty Liver: What the Research Shows

The picture for semaglutide fatty liver treatment is just as encouraging.

In the phase 3 ESSENCE trial, published in 2025, about 63% of people on semaglutide 2.4 mg had their steatohepatitis resolve without worse scarring, compared with around 34% on placebo. Roughly 37% saw their liver scarring improve, against about 22% on placebo. People on this treatment also lost more weight and had better liver health markers.

It was a landmark result. For the first time, a weight management medicine of this kind showed in a large phase 3 trial that it could reverse both inflammation and scarring in MASH.

One important point: neither treatment is currently licensed here purely to treat liver disease. Most of the liver benefits come from the weight loss these medications promote, although researchers are rapidly exploring their liver-specific uses.

How These Treatments Support Liver Function

To understand the benefit, it helps to look at these treatments together.

A fatty, inflamed liver struggles to do its many jobs, from filtering the blood to managing fat and sugar. These treatments ease this strain in several ways:

  • They drive weight loss, which directly lowers the fat stored in the liver.
  • They improve the body's ability to handle insulin, a key driver of fatty liver.
  • They help the liver burn fat rather than store it.
  • They often bring raised liver enzymes back towards normal as the liver recovers.

Most Wegovy liver side effects are uncommon, and the majority of side effects from these injections affect the gut rather than the liver itself.

  • Gallstones or gallbladder inflammation, which become more likely with fast weight loss.
  • Very rarely, drug-induced liver injury, a reaction in the liver itself.

The more common, day-to-day side effects involve the digestive system. These include feeling sick, being sick, diarrhoea, constipation, and a reduced appetite. They tend to ease as your body adjusts to the dose.

Can Mounjaro Cause Liver Damage?

The reassuring answer is that serious liver injury is rare. In the large trials, raised liver enzymes were no more common with these drugs than with a placebo, and clinically obvious liver injury was very unusual. A handful of case reports do describe a sudden, idiosyncratic reaction in the liver, but these cases are isolated, and most people will never experience anything like them.

It also helps to know that many liver complaints linked to weight loss injections actually come from the gallbladder, not the liver. Rapid weight loss can trigger gallstones, which cause pain in the upper stomach.
